CY06 Reporting

Greetings CSS on-campus Project Leaders,

Attached is a cover letter that lays out the procedure for CY06 faculty evaluations, a guide sheet for completing the Periodic Review of Faculty (PROF) form and a revised PROF. Please read the cover letter first followed by the guide sheet. The PROF should then make reasonable sense.

As indicated in the cover letter, you are to submit the completed PROF, a 2-4 page current vita (last five years of activity in detail, previous years summarized), and a current position description in electronic format to BARB REED by February 28, 2007. We would also appreciate having a current complete vita for award nomination and other uses, but this is optional. Check with Barb to see which position description she has on file. She may already have the most recent version.

The 2-4 page vita can be in whatever format you currently have. The attached outline lists desired format and information if you are looking for guidance.
